Anyone traveling into Utah is being required to submit a travel declaration form when entering the state.

Utah Governor Gary Herbert has signed an executive order which will allow the Utah Department of Transportation to collect information from every motorist entering the state. A wireless emergency alert system near the state's borders will be used to notify motorists of the requirement to complete a travel declaration form online.

The governor said in a statement, “We need to limit our travel to essential purposes only. Our goal is to trace potential cases of COVID-19 in inbound travelers.” The measure is designed to help the state "locate and isolate individuals who have been infected."

The executive order cites the Utah state code, which authorizes the governor to "control ingress and egress to and from a disaster area". The order specifies the disaster area in this case as "the entire state of Utah." The travel declaration is effective until May 1.

Upon crossing the Utah border, every motorist will receive a text message notifying them of the need to fill out the declaration form within three hours of entering the state. Information requested will include destinations traveled to in the previous 14 days, COVID-19 related health information, as well as other personal information.

You're not going to be stopped at the border when you head into Utah, but you are going to be expected to complete the form in a timely manner. The official order does not specify a penalty for those who fail to comply.
